OPT Mid-Term Grad (MTG) Extension enrollments are now available to purchase


 Be prepared to provide JCB with one of the following documents:

*Copy of Form I-797 USCIS Notice Receipt 

*Copy of OPT EAD card before completing the on-line enrollment


OPT students to purchase this plan, you must meet all the following eligibility requirements:

*You must be currently enrolled in the student health plan for this university (or) within 30 days of your plan termination date.

*You must have a valid EAD or I797 USCIS Notice Receipt – document upload is required.

*You must maintain continuous coverage – no gaps in coverage are allowed.


You are only eligible for the student insurance plan during your first 6 months of OPT – coverage beyond 6 months is not allowed.  For future sessions please check back for instructions on how to enroll prior to the termination date of the current session.

 * Your school has opted for a digital ID card. Your member ID card will be available on the plan start date. You’ll receive an email from Anthem Student Advantage once your ID card is ready to download. You’ll be able to email or fax it from your smartphone or computer. You can request a hard copy ID card by contacting Member Services at 1-800-888-2108.


Find a Doctor or Hospital

To find your EPO Network List of doctors, Hospitals and Urgent Care Centers click here, then enter your school, and "Find Care Here" option.

If you have a life threatening emergency, go to the Emergency Room or call 911. The Emergency Room usually sees the most urgent patients first. If it is urgent but not life threatening, you can go to an urgent care center which will usually cost you less money.

Student Health Services

 

Your campus health services is a great place to go for non-urgent medical services and prescriptions.  Health Services can treat many conditions or refer you to an off campus doctor or specialist, if necessary.  Contact them for more information about the services offered on campus: CLICK HERE FOR WEBPAGE or call (657) 278-2800

Registering with Anthem.com is simple

Use Anthem.com to keep track of your Student Health plan benefits and information - all in one place. Here are the simple steps to register and log into Anthem.com:

1. Navigate to Anthem.com

2. Once on the main page, click login on the top page

3. If you already have a registered account, click login and enter your username and password. If you need to register, click register now on the bottom of the page.

4. For SHIP plans, click student ID as the option for registering

5. Enter your student ID number, name and date of birth. Please make sure to enter your name as when you registered with the school

Once registered, you will need to verify and set up security settings

3 Important Enrollment Information

You must meet eligibility and attendance requirements set by your school to purchase or waive this insurance coverage. The insurance company and/or Gallagher Insurance, Risk Management and Consulting (the "Company") maintains the right to investigate student (and dependent) status and attendance records to verify that the policy eligibility requirements have been met. If and whenever the Company discovers that the policy eligibility requirements have not been met, our only obligation is to refund premium payment, minus any claims paid. By enrolling in this plan you are agreeing to your insurance carrier’s binding arbitration policies listed in the plan brochure. It is a crime to knowingly provide false, incomplete, or misleading information to an insurance company for the purposes of defrauding the company. Penalties include imprisonment, fines, and denial of insurance benefits. If your University is located in Washington State, the definition of Spouse includes Washington State Registered Domestic Partner.

*All refund requests must be sent to the University who will confirm nonstudent status with JCB, and submit the refund request on behalf of the student. The refund deadline is 45 days after the Insurance term start date. Only refunds submitted by the University before the refund deadline will be considered. Credit card refunds must be requested within 120 days of the date of purchase and before the refund deadline. No refunds will be considered after the refund deadline. All refunds will incur a $35 processing fee. *Does not apply to University of Michigan.
