All F-1 students are required to purchase the Fresno State University Student Health Insurance Plan before class registration.  


Create a Gallagher Student Account and log in to access important plan information and enroll in the plan.


YOUR ANTHEM MEDICAL ID CARD WILL NOT BE AVAILABLE UNTIL THE INSURANCE START DATE, OR 3 WEEKS AFTER ENROLLMENT, WHICHEVER IS LATER.

Get your ID Card

Mobile ID Cards will NOT be available until the term start date, or 3 weeks after enrolling in the plan, whichever is later. Access your ID card with Sydney Health app… With the Sydney Health app, you have instant access to:

Your member ID card, The Find a Doctor tool, More information about your plan benefits, Health tips that are tailored to you, LiveHealth Online, and 24/7 Nurse Line Student support specialists (through click-to-chat or by phone) From your mobile device or tablet go to the App Store or Google Play and search for the Sydney Health app to download it today.  * Your school has opted for a digital ID card. Your member ID card will be available on the plan start date. You’ll receive an email from Anthem Student Advantage once your ID card is ready to download. You’ll be able to email or fax it from your smartphone or computer. You can request a hard copy ID card by contacting Member Services at 1-800-888-2108.


Find a Doctor or Hospital

To find your PPO In-Network List of doctors, Hospitals and Urgent Care Centers click here Visit Anthem Student Advantage. You can also search for doctors, hospitals or urgent care centers on the Sydney Health App. From your mobile device or tablet go to the App Store or Google Play and search for the Sydney Health app to download it today. Questions about the app, please visit https://studentca.anthem.com/welcome or call Anthem Student Advantage at: 1-800-888-2108. 

If you have a life-threatening emergency, go to the Emergency Room or call 911. The Emergency Room usually sees the most urgent patients first. If it is urgent but not life threatening, you can go to an urgent care center which will usually cost you less money. 


Dependent Insurance

Dependent insurance is not available on the Fresno State sponsored plan. 

You may search online to find several insurance options or you can find a few by Clicking Here.  

Purchase coverage for your dependents online at your own risk. GSH and Fresno State maintain no association with or responsibility for, any Non-Fresno State plans purchased and will not provide enrollment and or support associated with any Non-Fresno State plans.


Student Health and Counseling Center

Your campus health center is a great place to go for non-urgent medical services and prescriptions. The Health Center can treat many conditions or refer you to an off-campus doctor or specialist, if necessary. Contact them for more information about your Student health & counseling center:

Dental Savings Program

Students enrolled in the school's Student Health Insurance Plan have access to a discount dental program that is available through UNI-CARE. This is not a dental insurance plan; there is no waiting period.  The UNI-CARE savings program provides you with a wide range of dental discount services. Savings vary but can be as high as 50% depending on the type of service received.

 

 How to Save: 

  • To find a participating provider go to findbestbenefits.com/student, choose Dental, and enter your zip code
  • Give the dental network name (UNI-CARE) to your provider when making an appointment
  • Download your membership card and present it to the provider prior to treatment to ensure proper discount is applied
  • Payment is due at time of service

Vision Savings Program

Students enrolled in the school’s Student Health Insurance Plan have access to a discount vision program that is available through Coast to Coast Vision. This is not a vision insurance plan; there is no waiting period.

Coast to Coast’s network has over 20,000 independent providers and retail stores nationwide, including For Eyes Optical, Lens Crafters, Pearle Vision, Target Optical, JC Penney Optical and Visionworks locations. Students save 10% to 60% off eyeglasses, contact lenses and other retail eyewear items, as well as 10% to 30% off eye examinations at any participating Coast to Coast Vision provider and 40% to 50% off the national average on LASIK.

How to Save:

Guidance Resources Online

If you are enrolled in your school's Student Health Insurance Plan, you will have access to Guidance Resources Online. Guidance Resources offers online self-guided programs designed for young adults to address anxiety, depression, stress, resilience, or insomnia. Based on cognitive behavioral therapy principles, these self-guided programs are available any time, on any device.

2 Important Enrollment Information

You must meet eligibility and attendance requirements set by your school to purchase or waive this insurance coverage. The insurance company and/or Gallagher Insurance, Risk Management and Consulting (the "Company") maintains the right to investigate student (and dependent) status and attendance records to verify that the policy eligibility requirements have been met. If and whenever the Company discovers that the policy eligibility requirements have not been met, our only obligation is to refund premium payment, minus any claims paid. By enrolling in this plan you are agreeing to your insurance carrier’s binding arbitration policies listed in the plan brochure. It is a crime to knowingly provide false, incomplete, or misleading information to an insurance company for the purposes of defrauding the company. Penalties include imprisonment, fines, and denial of insurance benefits. If your University is located in Washington State, the definition of Spouse includes Washington State Registered Domestic Partner.

*All refund requests must be sent to the University who will confirm nonstudent status with JCB, and submit the refund request on behalf of the student. The refund deadline is 45 days after the Insurance term start date. Only refunds submitted by the University before the refund deadline will be considered. Credit card refunds must be requested within 120 days of the date of purchase and before the refund deadline. No refunds will be considered after the refund deadline. All refunds will incur a $35 processing fee. *Does not apply to University of Michigan.
