OPT 1 (June Grad) enrollments are now available to purchase
Be prepared to provide JCB with one of the following documents:
- Copy of Form I-797 USCIS Notice Receipt
- Copy of OPT EAD card before completing the on-line enrollment
OPT students to purchase this plan, you must meet all the following eligibility requirements:
- You must be currently enrolled in the student health plan for this university (or) within 30 days of your plan termination date.
- You must have a valid EAD or I797 USCIS Notice Receipt – document upload is required.
- You must maintain continuous coverage – no gaps in coverage are allowed.
You are only eligible for the student insurance plan during your first 6 months of OPT – coverage beyond 6 months is not allowed.
For future sessions please check back for instructions on how to enroll prior to the termination date of the current session.

Find a Doctor or Hospital
To find your EPO Network List of doctors, Hospitals and Urgent Care Centers click here, then enter your school, and "Find Care Here" option.
If you have a life threatening emergency, go to the Emergency Room or call 911. The Emergency Room usually sees the most urgent patients first. If it is urgent but not life threatening, you can go to an urgent care center which will usually cost you less money.

Student Health Services
Your campus health services is a great place to go for non-urgent medical services and prescriptions. Health Services can treat many conditions or refer you to an off campus doctor or specialist, if necessary. LiveHealth Online
See a doctor on your computer or mobile device and get answers now. Plus online visits using LiveHealth Online are already part of your Anthem Blue Cross benefits. To start using LiveHealth Online, all you need to do is sign up at https://www.livehealthonline.com/ or download the app.
Registering with Anthem.com is simple
Use Anthem.com to keep track of your Student Health plan benefits and information - all in one place. Here are the simple steps to register and log into Anthem.com:
1. Navigate to Anthem.com
2. Once on the main page, click login on the top page
3. If you already have a registered account, click login and enter your username and password. If you need to register, click register now on the bottom of the page.
4. For SHIP plans, click student ID as the option for registering
5. Enter your student ID number, name and date of birth. Please make sure to enter your name as when you registered with the school

Dental Savings Program
Students enrolled in the school's Student Health Insurance Plan have access to a discount dental program that is available through UNI-CARE. This is not a dental insurance plan; there is no waiting period. The UNI-CARE savings program provides you with a wide range of dental discount services. Savings vary but can be as high as 50% depending on the type of service received.
How to Save:
- To find a participating provider go to findbestbenefits.com/student, choose Dental, and enter your zip code
- Give the dental network name (UNI-CARE) to your provider when making an appointment
- Download your membership card and present it to the provider prior to treatment to ensure proper discount is applied
- Payment is due at time of service

Vision Savings Program
Students enrolled in the school’s Student Health Insurance Plan have access to a discount vision program that is available through Coast to Coast Vision. This is not a vision insurance plan; there is no waiting period.
Coast to Coast’s network has over 20,000 independent providers and retail stores nationwide, including For Eyes Optical, Lens Crafters, Pearle Vision, Target Optical, JC Penney Optical and Visionworks locations. Students save 10% to 60% off eyeglasses, contact lenses and other retail eyewear items, as well as 10% to 30% off eye examinations at any participating Coast to Coast Vision provider and 40% to 50% off the national average on LASIK.
