OPT Trimester 2 is now closed
OPT Session 3 enrollments are now available to purchase.
Be prepared to provide JCB with one of the following documents;
copy of Form I-797 USCIS Notice Receipt, or copy of OPT EAD card before completing the on-line enrollment.
For OPT students to purchase this plan, you must meet all of the following eligibility requirements:
You must be currently enrolled in the student health plan for this university (or) within 30 days of your plan termination date.
You must have a valid EAD or I797 USCIS Notice Receipt – document upload is required.
You must maintain continuous coverage – no gaps in coverage are allowed.
You are only eligible for the student insurance plan during your first 12 months of OPT – coverage beyond 12 months is not allowed.
For future sessions please check back for instructions on how to enroll prior to the termination date of the current session.

Dependent Insurance is NOT available on the Cal Poly Pomona sponsored plan.
