NOTICE

If you enrolled using the CSULB address as your home address, you are required to return to your JCB student account at www.jcbins.com to update your information when you have your US address.

CSULB will automatically receive confirmation of your enrollment in the ALI Student Health insurance Plan. There is no need to submit your enrollment confirmation to CSULB

Get your GeoBlue ID card

ID cards are electronic only!  You will receive an email from GeoBlue at the start of each semester notifying you to download your ID card. 
You may also set up an account at www.geobluestudents.com to access your ID card any time.  It is important to have your GeoBlue ID card with you whenever you receive medical care.

Geo Blue Find a Provider

  • Visit www.geobluestudents.com
  • Register, or if you have already registered, login
  • Choose US Provider Finder on Member HUB
  • Launch the Blue National Doctor and Hospital Finder
  • Enter your local City and State
  • Enter the first 3 digits on your ID card (i.e.: QHS)
  • Search for providers by name or specialty
    • Family Practice, General Practice and Internal Medicine options will act as your primary care physicians
    • Specialists are focused on specific medical conditions
    • Urgent Care providers can see you for urgent needs that are not life threatening in nature
    • Hospital Emergency Rooms are for life threatening illness or injury

Student Health Services

Your campus Student Health Services (SHS) is a great place to go for non-urgent medical services and prescriptions. The SHS can treat many conditions or refer you to an off campus doctor or specialist, if necessary. Contact them for more information:

2 Important Enrollment Information

You must meet eligibility and attendance requirements set by your school to purchase or waive this insurance coverage. The insurance company and/or Gallagher Insurance, Risk Management and Consulting (the "Company") maintains the right to investigate student (and dependent) status and attendance records to verify that the policy eligibility requirements have been met. If and whenever the Company discovers that the policy eligibility requirements have not been met, our only obligation is to refund premium payment, minus any claims paid. By enrolling in this plan you are agreeing to your insurance carrier’s binding arbitration policies listed in the plan brochure. It is a crime to knowingly provide false, incomplete, or misleading information to an insurance company for the purposes of defrauding the company. Penalties include imprisonment, fines, and denial of insurance benefits. If your University is located in Washington State, the definition of Spouse includes Washington State Registered Domestic Partner.

*All refund requests must be sent to the University who will confirm nonstudent status with JCB, and submit the refund request on behalf of the student. The refund deadline is 45 days after the Insurance term start date. Only refunds submitted by the University before the refund deadline will be considered. Credit card refunds must be requested within 120 days of the date of purchase and before the refund deadline. No refunds will be considered after the refund deadline. All refunds will incur a $35 processing fee. *Does not apply to University of Michigan.
